Introduction Clostridium difficile, a Gram-positive spore bearing anaerobic bacteria is the major aetiological agent of diarrhoea and colitis associated with antibiotics. Hall and O'Toole (1) originally identified the organism as a component of normal colonic flora of newborn infants. C. difficile is commonly present in the stools of 5 per cent of healthy adults usually in low numbers (2) and in about 15-70 per cent of infants (3-5). The majority of hospitalized patients infected by C. difficile are asymptomatic carriers who serve as silent reservoirs for continued C. difficile contamination of the hospital environment (6). However C. difficile'-associated disease (CDAD) is a serious condition with mortality up to 25 per cent in frail elderly people (7). C. difficile is now recognized as the primary cause of hospital acquired colitis in patients who receive antibiotics, chemotherapeutics or other drugs that alter their normal flora.
